A severe illness can be devastating to a family’s finances. The Health Alliance is making available to you the Personal Recovery Plus Policy. This critical illness insurance provides financial assistance when you are treated for any of the following specified health events:

  • Heart Attack
  • Coma
  • Stroke
  • Paralysis
  • End stage renal failure
  • Major human organ transplant
  • Coronary artery bypass surgery
  • Major third degree burns

This insurance is “simplified issue” which means that you will be asked a short series of questions dealing with health issues covering the previous five years. Pre-existing condition limitations are applied to any illness or injury suffered during the six-month period prior to the plan’s effective date. Benefits are payable for pre-existing conditions if the specified health event occurs more than 30 days after the effective date of coverage.

Cost of Coverage
The cost of coverage varies depending on the individuals covered and their ages. Refer to the cost sheet for specific premium amounts. Premiums are deducted on a pre-tax basis from every paycheck. However, if your policy includes coverage for a domestic partner, deductions are taken on an after-tax basis.
