If you were already benefits eligible
- Confirm that your Supervisor has submitted an e-Status
form for you.
- Decide if you will need to raise your dependent
care Flexible Spending Account contribution.
- Consider if you need to purchase Supplemental Employee
Life insurance.
- Contact your Human Resources department for a Benefits
Enrollment & Change form and return it to your
Human Resources department before the end of 30 days
from the date your hours changed.

eligible)

If you are newly benefits eligible
- Confirm that your Supervisor has submitted an e-Status
Form for you.
- Read your Associate Benefits Booklet
- Consider your family circumstances and make careful
choices of the benefit plans you wish to enroll in.
- Fill out an Benefits Enrollment or Change form.
- Return the form to your Human Resources department
before the end of 30 days from the date your hours
changed and you became benefits-eligible.

- If you have Supplemental Life plan coverage, contact
Human Resources and submit a Benefits Change form
within 30 days of the event or you will lose your
coverage.
- Check your pay stub carefully for several pay periods
to determine that your benefits are correct.
