Non - Medical Leaves |
- Read the Leave of Absence policy.
- Leaves of 10 days or less (other than FMLA leaves)
must be approved by your supervisor.
- Leaves of more than 10 consecutive days (three for
Family Medical Leave), fill out a Request for Leave
of Absence and submit it to your supervisor.

Medical Leaves |
- Read the Leave of Absence policy.
- Submit a Request for Leave of Absence to your supervisor
or Employee Health at least 30 days prior to the start
of your leave. (In case of emergency, within one or
two days of learning of need.)
- Provide Employee Health with certification of need
for leave by your health care provider.
- Report leave status and intention to return to work
to supervisor every 30 days.
